Activity Coordinator

Estimated salary: $21.72 - $26.72 per hour.

We are seeking an experienced Activity Coordinator to join our team at Sunset Villa Post Acute SNF. The ideal candidate will be responsible for planning and conducting individual, small and large group activities, as well as facilitating effective communication between employees of all levels, residents, their families, support personnel, government agencies/personnel, and the public.

About the Role

The successful candidate will participate in community planning related to the interests of the facility and the services and needs of the resident and family.

  • Develop and implement a monthly activity calendar, maintaining accurate attendance records.
  • Contribute to discharge planning, development and implementation of activity care plans and resident assessments.
  • Arrange transportation for residents when necessary.
Key Responsibilities
  • Plan and conduct individual, small and large group activities that cater to diverse interests.
  • Maintain good relationships with employees, residents, and their families.
  • Collaborate with healthcare professionals to develop and implement care plans.
Requirements
  • Bachelor's degree or equivalent required.
  • Preferably one year experience in a long-term care facility.
  • Strong communication and interpersonal skills.
