About the Job

We are seeking an experienced Triage Nurse to join our team at NorthPoint Health & Wellness Center. As a Triage Nurse, you will play a critical role in evaluating patients' symptoms and conditions to determine the appropriate level of care. You will work under the direction of the nursing supervisor to provide triage services, assist with scheduling appointments, and complete case management and follow-up care.

Responsibilities
  • Provide triage services by evaluating patients' symptoms and conditions to determine the appropriate level of care.
  • Assist with scheduling appointments and complete case management and follow-up care.
  • Work with multiple providers, staff, and collaborative partners to develop and maintain a plan of care for diverse and underserved patients.
  • Communicate and carry out provider care orders.
  • Make or request referrals as appropriate.
  • Collect pertinent information and provide appropriate actions for patients who are seeking emergency care.
  • Complete patient interaction calmly and empathetically while multitasking critical thinking.
  • Provide case management, treatment follow-up, and medication refills according to predetermined standing orders.
  • Provide health education and counseling to clients on disease conditions and health maintenance.
Requirements
  • Graduation from an accredited school of nursing.
  • A valid license, in good standing, as a registered nurse, or a permit to practice, issued by the State of Minnesota.
  • Current CPR certification.
  • Three or more years of experience as a registered nurse in a health care setting, preferably primary care.
  • Basic Life Support (BLS) certification through the American Heart Association.
Preferred Qualifications
  • Experience providing face-to-face and phone triage in a clinical setting.
  • Experience using Epic and/or any other electronic health record system.
  • Experience working with multiple providers, staff, and collaborative partners with differing needs and opinions to improve the care to diverse, underserved, and at-risk populations with a variety of cultural and language barriers.
