Sign-On Bonus: $10,000 for experienced Hospital Social Workers for 0.75 FTE or larger.
Relocation Assistance: Up to $10,000 based on mileage, with a 2-year agreement to MultiCare.
Responsibilities- Conduct cross-continuum care activities, which may include clinic, provider practice, urgent care, phone care agency, skilled care, or other care settings as appropriate.
- Deliver exceptional customer service to patients, families, physicians, and care team members, ensuring smooth discharge processes and transitions to the next level of care.
- Exhibit and promote consistent, professional communication, maintaining proactive communication patterns with colleagues and staff.
- Delegate responsibilities effectively while remaining accountable for outcomes.
- Ensure that patients and families experience medically appropriate transitions of care.
- Master's degree in Social Work with a healthcare-based practicum.
- Licensed in the state of Washington as a Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within four years.
- Preferred experience in a hospital or equivalent setting; prior experience before obtaining a Master's degree may be considered.
- Preferred knowledge of the health industry and healthcare financial assistance.
