Work Schedule: Monday-Sunday, variable days, alternating weekends
Position Overview:
This role involves delivering a comprehensive array of clinical social work services within a primary care or home health environment. The professional will provide support and guidance to the care team in managing patient needs. The evaluation of psychosocial strengths and challenges is essential to assist patients in achieving the highest level of independence. Collaboration with other professionals is crucial to develop a cohesive care plan. The position also includes offering short-term counseling, educational support, and connecting families with additional resources for caregiving.
Key Responsibilities:
- Deliver tailored social work interventions to patients of all ages and their families, which encompass psychosocial assessments, brief individual or family counseling, advocacy, resource linkage, referral services, group facilitation, case management, and long-term care planning.
- Engage in consultation and coordination with a multidisciplinary team of healthcare professionals and primary care medical staff, participating in family meetings, care discussions, and team collaborations.
- Assess patient and family requirements for care, ensuring effective communication with other healthcare providers. Collect and analyze relevant data throughout the care process, continuously evaluating psychosocial strengths and challenges to support patient independence.
- Conduct timely evaluations of patient and family needs through assessments, communication, and observation, adjusting care plans based on patient responses and care system dynamics.
Qualifications: Experience
- At least one (1) year of prior social work experience is required.
- Current employees may have this experience requirement waived as per the National Agreement.
- A Master's degree in Social Work (MSW) is mandatory.
- Must possess one of the following licenses: Licensed Marriage and Family Therapist (Washington), Licensed Advanced Social Worker License (Washington), Licensed Social Worker Associate-Independent Clinical (Washington), Licensed Independent Clinical Social Worker (Washington), or Mental Health Counselor License (Washington).
- Strong written and verbal communication skills are essential.
- Proven ability to work effectively within a multidisciplinary team and a commitment to fostering diversity.
- Two (2) years of post-master's experience is preferred.
- Experience in care coordination within managed care systems, inpatient/outpatient settings, skilled nursing, or long-term care facilities is advantageous.
- Knowledge of social case management, conflict resolution, and team-building strategies is beneficial.
- Proficiency in Microsoft Office applications is preferred.
- Experience with electronic charting systems is an asset.
