Position Overview:
This role involves delivering a comprehensive array of clinical social work services within a primary care or home health environment. The incumbent will provide support and guidance to the care team in managing patient needs. Responsibilities include evaluating psychosocial strengths and challenges to empower patients in achieving optimal independence. Collaboration with various stakeholders to develop a tailored care plan is essential. The position also encompasses short-term counseling, educational support, and resource navigation for caregiving requirements.
Key Responsibilities:
- Implement suitable social work interventions for patients of all ages and their families, which may include psychosocial assessments, brief individual or family counseling, advocacy, resource linkage, referrals, group facilitation, case coordination, and long-term care planning.
- Engage in consultation and coordination with a multidisciplinary team of healthcare professionals and primary care staff, participating in family and care conferences, team meetings, and similar collaborative efforts.
- Assess patient and family needs for care, effectively communicating with other healthcare providers. Gather relevant data during visits and conduct ongoing assessments throughout the care process. Evaluate psychosocial strengths and challenges to support patient independence.
- Evaluate the needs of patients and families through timely assessments, communication, and observation. Adjust care plans based on patient and care system responses.
Qualifications: Experience
- At least one (1) year of prior social work experience.
- Current employees may have this experience requirement waived as per the National Agreement.
- Master's degree in social work (MSW).
- Licensed Marriage and Family Therapist (Washington) OR Licensed Advanced Social Worker License (Washington) OR Licensed Social Worker Associate-Independent Clinical (Washington) OR Licensed Independent Clinical Social Worker (Washington) OR Mental Health Counselor License (Washington)
- Exceptional written and verbal communication abilities.
- Proven success in a multidisciplinary team environment and a commitment to diversity and inclusion.
- Two (2) years of post-master's experience.
- Experience in care coordination within managed care systems, inpatient/outpatient settings, skilled nursing, or long-term care facilities.
- Understanding of social case management, conflict resolution, and team-building practices.
- Proficiency in Microsoft Office applications.
- Experience with electronic health record systems.
