Key Responsibilities- Provide case finding and intake services to identify patient needs and develop personalized care plans.
- Collaborate with healthcare providers to ensure seamless care coordination and outcomes evaluation.
- Manage daily utilization activities to optimize patient care and reduce costs.
- Address psychosocial and economic issues affecting patient well-being and quality of life.
- Graduate from an accredited school of nursing (RN) required, bachelors preferred.
- Two (2) or more year's clinical experience preferred, with acute care or community social agency case management experience.
- Must possess good assessment, intervention, and problem-solving skills, with ability to communicate effectively and efficiently.
- Basic computer skills, including Outlook, Word, Excel, and databases, are a must.
- Ability to work with culturally diverse populations and maintain collaborative relationships with multiple stakeholders.
