Job Title: Case Manager

Job Summary:

The Case Manager is a vital member of our team at House of Ruth, responsible for developing and implementing therapeutic relationships with clients, assessing their strengths and needs, and creating personalized case management plans. This role requires strong analytical, planning, and organizing skills, as well as the ability to work effectively with cross-functional teams and communicate with clients at various levels.

Key Responsibilities:

  • Orient new participants and establish regular meetings with clients in the office and community.
  • Develop a therapeutic relationship, assess strengths and needs, and formulate a case management plan.
  • Complete and maintain accurate documentation of case management and counseling activities.
  • Communicate essential issues to relevant contacts and gather information from other sources.
  • Identify resources and link clients to comprehensive services, advocating for services as needed.
  • Establish and maintain professional contacts with House of Ruth staff and other organizations to ensure coordinated services.
  • Accompany and transport clients to appointments as needed.
  • Develop and conduct psycho-education and support groups, teaching living skills and social skills.
  • Provide positive role modeling, supportive counseling, and crisis intervention sessions.
  • Ensure clients adhere to program expectations and address instances of non-adherence.
  • Maintain a safe and secure environment for all occupants.
  • Effectively manage emergencies by following standard procedures.
  • Monitor medications, conduct urine toxicology screens, and complete related documentation.
  • Screen telephone calls and walk-ins, provide crisis intervention, and make referrals.
  • Attend training sessions and meetings.
  • Complete the initial assessment process for prospective clients.
  • Provide on-call support and ongoing briefings to the program coordinator.

Requirements:

  • Bachelor's Degree in Social Work or related field required.
  • 1-2 years' experience in a case manager role.
  • Experience working in a residential setting preferred.
  • Strong analytical, planning, and organizing skills.
  • Self-starter with good time management skills.
  • Strong working knowledge of MS Office 365.
  • High level of interpersonal skills to handle sensitive situations.
  • Excellent written and verbal communication skills.
  • Good problem-solving skills.
  • Team player with demonstrated ability to work with cross-functional teams.
  • Ability to communicate and present across levels up to executive leadership.
  • Demonstrated ability to teach others.
  • Must be willing to travel up to 25% throughout the DC area.
  • Must be able to work a flexible schedule due to the 24/7 operation of House of Ruth programs.
  • Exhibit a passion for working with women and/or children.
