Job Title: Case Manager

House of Ruth is seeking a skilled Case Manager to join our team. The successful candidate will be responsible for developing a therapeutic relationship with clients, assessing their strengths and needs, and creating a case management plan to support their goals.

Key Responsibilities:

  • Orient new participants and meet regularly with clients in the office and in the community.
  • Develop a therapeutic relationship, assess strengths and needs, and formulate a case management plan.
  • Complete and maintain accurate documentation reflecting all case management and counseling functions and activities.
  • Communicate essential issues to relevant contacts and gather information from other sources.
  • Identify resources and link adults and children to a comprehensive set of services.
  • Advocate for services to assist families.
  • Establish and maintain professional contacts with House of Ruth staff and staff from other organizations.
  • Accompany and/or transport clients to appointments as needed.
  • Develop and conduct psycho-education and support groups and activities.
  • Provide positive role modeling and supportive counseling.
  • Ensure families adhere to program expectations and address instances of non-adherence.
  • Maintain a safe, secure, and non-violent environment for all occupants.
  • Effectively manage emergencies and follow standard procedures.
  • Monitor medications and conduct urine toxicology screens.
  • Screen telephone calls and walk-ins and provide crisis intervention.
  • Attend training sessions and meetings.
  • Complete the initial assessment process of perspective clients.
  • Provide on-call support and ongoing briefings to the program coordinator.

Requirements:

  • Bachelor's Degree in Social Work or related field.
  • 1-2 years' experience in a case manager role.
  • Experience working in a residential setting preferred.

Preferred Qualifications:

  • Strong analytical, planning, and organizing skills.
  • Self-starter with good time management skills.
  • Strong working knowledge of MS Office 365.
  • High level of interpersonal skills to handle sensitive and confidential situations.
  • Excellent written and verbal communication skills.
  • Good problem-solving skills.
  • Team player with demonstrated ability to work with cross-functional teams.
  • Ability to communicate and present across levels up to executive leadership.
  • Demonstrated ability to teach others.
  • Must be willing to travel up to 25% throughout the DC area.
  • Must be able to work a flexible schedule due to the 24/7 operation of House of Ruth programs.
  • Exhibit a passion for working with women and/or children.
