Job Overview

At Youth and Family Alternatives, Inc., we are dedicated to providing a safe and nurturing environment for children in crisis. As a Case Manager, you will play a critical role in supporting our mission by providing continuity of care and ensuring the well-being of our youth.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each child.
  • Ensure the psycho-social well-being of our youth by providing counseling and support services, as well as connecting them with community resources.
  • Collaborate with internal and external stakeholders to ensure compliance with clinical contracts and regulatory requirements.
  • Provide case management services, including arranging informal and formal contacts with relevant individuals and maintaining communication with visiting teachers.
  • Document all interactions and progress in a comprehensive and accurate manner.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a related field, such as human services or counseling, or a Master's degree with relevant experience.
  • At least 4 years of post-Bachelor's experience in human services, preferably in counseling or case management.
  • Ability to travel regionally and statewide as needed, as well as lift and carry up to 30 pounds.
