Job Description

Caregivers at Hearts and Hands of Care play a vital role in supporting individuals with unique needs in their communities or homes. Our team members are responsible for providing ongoing services, ensuring the safety and well-being of our clients, and documenting daily progress. We are seeking a dedicated and compassionate individual to join our team as a Direct Support Professional.

Key Responsibilities:

  • Provide high-quality care and support to individuals with varying needs
  • Implement and maintain individualized service plans
  • Document daily progress and maintain accurate records
  • Communicate effectively with team members, supervisors, and clients
  • Participate in ongoing training and professional development

Requirements:

  • High school diploma or equivalent required
  • Experience working with individuals with unique needs preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Reliable transportation and a valid driver's license
