Case Manager III

City of Norfolk, VA is seeking a highly skilled Case Manager III - DD Support Coordinator to join our team. As a key member of our Community Services Board, you will play a vital role in providing treatment, support, and education to individuals with intellectual and developmental disabilities.
Job Summary:The Case Manager III - DD Support Coordinator will assist individuals with IDD in accessing necessary medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community. This includes developing individual support plans, linking, coordinating, and monitoring services, and ensuring compliance with agency guidelines and state regulations.
Responsibilities:- Conduct initial and annual assessments of individuals to plan services and supports
- Develop and implement individual support plans in collaboration with individuals and/or families
- Link, coordinate, and monitor services, including Medicaid Waiver Services provided by private providers, other community agencies, natural support systems, and other units in the NCSB
- Maintain chart documentation in compliance with agency guidelines and state regulations
- Ensure documentation substantiates billing for services
- Comply with all State Case Management Licensure regulations, HIPAA, and Human Rights regulations
- Conduct face-to-face visits at a minimum of every 90 days and complete monthly activity related to the individual service plan
- Monitor progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload
- Identify and access resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements
- Bachelor's degree in a human services field
- Two years of experience in a human services field related to the areas of assignment
- Valid Driver's License may be required depending on assignment
- Certification in basic first aid and CPR, depending on area of assignment
City of Norfolk, VA offers a competitive salary range of $46,589-$53,577, a one-time $5,000 signing bonus for new hires, and a comprehensive benefits package, including paid holidays, vacation, and sick leave, employer-paid pension plan, basic life insurance, voluntary participation in medical and dental, Section 457 deferred compensation, long-term disability, optional life insurance for self, spouse, and children, medical and dependent care reimbursement plans, access to membership in credit union, and employer-provided parking.
