Overview:


The Patient Care Coordinator plays a vital role in assisting patients, families, and caregivers in navigating the complexities of the healthcare system, ensuring they receive timely access to high-quality medical and psychosocial support.

This role encompasses all stages of the cancer journey, from prevention and screening to diagnosis, treatment, survivorship, and end-of-life care.

The Coordinator provides educational resources and supports patients and caregivers in completing necessary health-related documentation under the guidance of the Oncology Nurse Navigator. This position relies on established educational frameworks, clinical guidelines, and legal standards to fulfill its responsibilities.

Responsibilities:
Key Responsibilities

Collaborates closely with the Oncology Nurse Navigator to manage patient care throughout the entire continuum

Informs patients, families, and caregivers about the Patient Care Coordinator's role in:

  • Facilitating communication among healthcare professionals
  • Assisting patients and families with medical documentation
  • Coordinating essential clinical appointments
  • Providing information on available resources

Supports the Oncology Nurse Navigator in identifying patient and family challenges and exploring potential solutions, while respecting the patient's values and cultural, spiritual, and social considerations

Coordinates patient appointment needs and schedules efficiently to enhance the care plan

Ensures all referral processes, both internal and external, are completed promptly by securing and including referral documentation in the medical record

Accurately documents all patient interactions and services rendered within the electronic medical record system

Acts as an advocate and liaison for patients, families, and caregivers, facilitating communication with the multidisciplinary team

Serves as a point of contact for referring physicians and external medical offices, ensuring timely and professional communication

Engages in ongoing process improvement initiatives to enhance patient outcomes, ensuring effective use of resources to boost patient, family, and caregiver satisfaction

Maintains patient confidentiality in accordance with local and national regulations, treating all patient information as privileged and protected

Participates in events sponsored by the oncology network to share insights and best practices in patient navigation

Adheres to compliance programs, including ethical standards and business practices.

Minimum Requirements:
High school diploma or GED; advanced education is preferred

A minimum of four (4) years of experience in a clinical healthcare setting is required

Experience as a certified Medical Assistant with a focus on oncology/hematology patient populations is preferred

Experience in oncology-related education, planning, or management is essential

Critical Skills:
Excellent organizational and interpersonal abilities

Proficient in using electronic medical record (EMR) systems

Skilled in reviewing medical records and patient care information

Strong understanding of HIPAA compliance and patient confidentiality regulations

Ability to prioritize tasks effectively

Capability to foster collaborative relationships and work within teams, including oncologists and referring providers

Strong written and verbal communication skills, including familiarity with medical terminology and therapeutic options

Ability to work independently with minimal supervision

Additional Knowledge/Skills:

Exceptional customer service skills

Comprehensive knowledge of oncology patient care and outpatient workflow

Intermediate computer skills, including proficiency in MS Office applications and internet research

Awareness of local community resources.
