Jaan Health, Inc. is seeking a skilled Chronic Care Coordinator to join our team at Partners In Nephrology & Endocrinology. As a Chronic Care Coordinator, you will play a vital role in supporting the delivery of high-quality, patient-centered care to individuals with Chronic Kidney Disease (CKD).
Key Responsibilities- Manage a panel of chronic care patients using the Phamily platform, providing support to nephrologists and advanced practitioners in the delivery of care.
- Facilitate timely access to and provision of preventive services and chronic disease treatment, with the goal of improving patient outcomes.
- Bridge the communication gap between healthcare providers and patients, reducing unnecessary hospitalizations and improving overall health.
- Minimum of 1 year experience as a Certified Medical Assistant, with experience in population health preferred.
- Proven problem-solving skills, with the ability to multitask and prioritize tasks effectively.
- Excellent communication skills, both written and spoken, with the ability to effectively communicate with patients, healthcare providers, and other stakeholders.
- Prior experience with Epic or other electronic health record systems.
- Experience working in nephrology or a related field.
- Prior experience as a Care or Case Coordinator.
- Competitive hourly rate of $18-$19.50.
- Standard Monday-Friday schedule, with 8:00 AM - 5:00 PM hours.
