AltaStaff is a staffing agency currently seeking a Registered Nurse Care Manager (UAS) to collaborate with our Managed Care Client.

Location: Syracuse, NY.

Bilingual candidates in any language are highly preferred, but monolinguals are also welcome.

Compensation: $56.00 - $58.00 per hour

Work Schedule: Monday - Friday; 8:30 AM - 5 PM EST

This position is hybrid and requires travel to member residences.

Mileage reimbursement is provided.

Overview: Candidates must possess a valid NY RN license to carry out case management responsibilities. RN's will receive training on interpreting UAS assessments and will conduct visits to members in their homes, assisted living facilities, and nursing homes. Daily tasks will include performing UAS Assessments to facilitate care management, with a minimum of 15 cases assigned weekly.

Key Responsibilities:

  • Conduct in-person evaluations of members' functional status, medical, behavioral, psychosocial, and community resource needs.
  • Provide the Interdisciplinary Care Team (ICT) with assessment data and act as a facilitator to ensure that members' needs are met.
  • Develop an Individualized Care Plan/Person-Centered Service Plan (ICP/PCSP) based on members' clinical, behavioral, and social needs, addressing barriers to care.
  • Effectively assess members' health status and ensure they receive all necessary medical and supportive services.
  • Adjust the ICP/PCSP as needed based on the member's requirements and progress.
  • Manage care transitions through effective and timely communication essential for member care and discharge planning.
  • Clarify medical benefits, policies, and procedures for members, providers, and community-based agencies.
  • Prepare for and engage in clinical case reviews to share best practices.
  • Adhere to documentation policies and procedures.
Position Qualifications:
  • Valid NYS RN Licensure is required.
  • Bilingual in Spanish or Bengali is required.
  • Strong professional knowledge and comprehensive clinical assessment skills in adult populations and chronic disease management.
  • Excellent communication abilities.
  • Proficient in Microsoft Office Suite (Outlook, Word, Excel).
  • Ability to work independently and adapt in a fast-paced environment.
  • Capability to analyze data and utilize it to enhance care delivery.
  • Self-motivated with a high level of accountability for care outcomes.
  • Highly organized and able to manage multiple priorities effectively.
  • Independent problem-solving skills and the ability to collaborate and build lasting relationships with providers, members, and the multidisciplinary team.
Work Environment Requirements:
  • Must be able to lift at least 30 lbs.
  • Must be able to stand or sit for extended periods (75% of the time).
  • Private secured internet connection is necessary.
  • Must ensure a safe and secure work environment for handling private and sensitive information.
  • Constantly operates a computer and other office equipment.
  • Works indoors in a secluded and safe space.
  • High-speed internet access (minimum 25 MBPS; 50 MBPS preferred) is required.
