Position Overview:

The RN Care Coordinator will oversee all systems and services necessary for a structured, multidisciplinary, patient-centered care team approach, ensuring quality and cost-effective care for the designated patient population.

This role involves managing patient treatment plans by coordinating care with physicians, nurses, and other healthcare professionals, while ensuring optimal patient outcomes are achieved within set timelines and through efficient resource utilization.

The RN Care Coordinator will interact with community providers, insurance companies, and agencies throughout the care continuum. Additionally, this position will supervise the work of LPN Care Coordinators and participate in Patient-Centered Medical Home initiatives.


Minimum Qualifications:
- Current RN Licensure in New York
- Associate's Degree in Nursing; Bachelor's Degree in Nursing (BSN) preferred
- Certification in Case Management (CCM, ACM) preferred
- Familiarity with the provider community and resources in the Central New York area
- Self-motivated, enthusiastic, and innovative
- Strong communication skills to engage with community healthcare organizations
- Ability to maintain confidentiality in accordance with Practice, State, and Federal regulations
- Demonstrated sound judgment, decision-making, and problem-solving abilities
- Highly organized with well-developed oral and written communication skills
- Comprehensive understanding of insurance principles and outpatient delivery systems

Preferred Qualifications:
- A minimum of 5 years of clinical experience

Note:
This is a remote position; however, candidates must reside in Central New York.

Compensation:
$58,000 - $72,000.00 based on experience.
