Oncology Care Coordinator

About the Role
As a Oncology Care Coordinator at Montefiore St. Luke's Cornwall, you will play a vital role in coordinating the care and services of patients across their cancer journey. This interdisciplinary position involves assessing, planning, implementing, monitoring, and measuring the effectiveness of interventions to meet patients' treatment and discharge needs.
Key Responsibilities
- Coordinate patient care across the continuum of their cancer journey
- Collaborate with healthcare teams to assess and develop individualized care plans
- Ensure timely and effective implementation of care plans
Requirements
- Master's degree in Social Work from an accredited school of Social Work
- Minimum one year experience in oncology preferred
- Strong clinical, organizational, and interpersonal skills
- Ability to read, write, and communicate in English
