Role Responsibilities
In this role, you will utilize your specialized care plans and serve as an innovator in patient care. Your contributions will shape exceptional patient experiences daily, leveraging your expertise and advanced technology to enhance patient outcomes.

Qualifications
Applicants must possess a current state RN license and an associate's degree from an accredited nursing institution. Additional qualifications include:
- BLS certification for Healthcare Providers upon hire
- Minimum of one year of acute care experience
- BSN is preferred
- Experience in wound care and ostomy is preferred
- Wound care/Ostomy certification is preferred
Department Summary
The RN Nurse Navigator role focuses on planning, coordinating, and delivering specialized care in wound, ostomy, and continence management. You will collaborate with the wound care team to provide education to patients and their families, standardizing care for those with critical limb ischemia. Responsibilities include conducting risk assessments, facilitating necessary testing, and serving as a resource for nursing practices related to skin, wound, ostomy, and continence care. You will also implement a comprehensive pressure sore prevention program and coordinate ongoing education for staff.
