Qualifications
Applicants must possess a current state RN license and an associate's degree from an accredited nursing institution. Additional qualifications include:

  • BLS certification for Healthcare Providers upon hire
  • Minimum of one year of acute care experience
  • BSN is preferred
  • Experience in wound care and ostomy is preferred
  • Wound care/Ostomy certification is preferred

Department Overview

The RN Nurse Navigator role focuses on planning, coordinating, and delivering specialized care in wound, ostomy, and continence management. You will standardize care for patients with critical limb ischemia, perform risk assessments, facilitate necessary testing, and educate patients and their families regarding their conditions and procedures. Additionally, you will serve as a resource for nursing practices related to skin, wound, ostomy, and continence care within the hospital and community. Responsibilities also include coordinating a comprehensive pressure sore prevention program and ongoing education for staff in all aspects of skin and wound care.
