Job Summary

The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting, offering therapeutic plans utilizing best evidence and most effective medication treatments. This role involves interactions with patients, providers, and interdisciplinary teams to achieve positive patient outcomes.


Key Responsibilities

  1. Design, implement, assess, monitor, and document therapeutic plans.
  2. Provide education to patients and caregivers, and document care plans in the medical record.
  3. Order, interpret, and monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease.
  4. Support the quality of care in ambulatory practice areas, enhance patient access to affordable care, and engage in professional self-development.
  5. Perform patient assessments, participate in collaborative practice agreements, and provide medication management.

Requirements

  • Bachelor's in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy.
  • PGY-1 residency required or equivalent clinical experience.
  • Commonwealth of Virginia State Board of Pharmacy License required.
  • American Heart Association (AHA) Health Care Provider BLS certification required.
