What You Will Do

As a Case Manager with Telecare Corporation, you will play a vital role in supporting individuals with substance use and mental health challenges. Your expertise will help clients navigate the complexities of their conditions, providing them with the tools and resources needed to achieve stability and independence.

Key Responsibilities

  • Develop and implement personalized care plans to address clients' unique needs
  • Collaborate with healthcare professionals, family members, and community resources to ensure comprehensive support
  • Provide education and guidance on healthy coping mechanisms, stress management, and relapse prevention
  • Conduct regular assessments and evaluations to monitor clients' progress and adjust care plans as needed
  • Document client interactions, progress, and challenges in a timely and accurate manner

Requirements

To succeed in this role, you will need:

  • A degree in a related field, such as social work, counseling, or psychology
  • Experience working with individuals with substance use and mental health challenges
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Proficiency in electronic health records and other relevant software
