Care Manager RN

As a key member of the Elder at Home care team, the RN-Care Manager plays a vital role in providing comprehensive care coordination services and managing transitions of care for patients with complex medical needs. This role requires a strong understanding of patient-centered care and the ability to develop and implement personalized care plans that promote self-management and independence in the home and community setting.

Key Responsibilities:
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Coordinate and facilitate transitions of care between healthcare settings, ensuring seamless and safe transitions for patients.
- Provide education and support to patients and their families, empowering them to take an active role in their care and self-management.
- Collaborate with healthcare providers and community resources to ensure comprehensive and coordinated care.
Requirements:
- Oregon Registered Nurse License upon hire.
- 2 years of clinical medical-surgical, acute care, or long-term care experience, including 1 year in community-based/ambulatory care and 1 year in care of the frail elderly.
Preferred Qualifications:
- Bachelor's Degree in Nursing.
- Experience in home health, hospice, palliative care, managed care, or skilled nursing facility settings.
- 1 year of care management experience with diverse patient populations.
