Job Summary

Sea Mar Community Health Centers is seeking a highly skilled Care Coordinator to join our team at the Puyallup Medical Clinic. As a key member of our clinical care team, you will play a vital role in enhancing quality and patient-centered care for our patients with chronic conditions and mental health needs.

Key Responsibilities:

  • Assess gaps in care and create plans with the clinical care team during daily huddles.
  • Assist patients with medication management, insurance access, and preventive health needs.
  • Facilitate team meetings and utilize Motivational Interviewing skills to support patient self-management.
  • Track patient adherence with electronic charts and communicate outcomes to primary care providers.
  • Collaborate with the care team to improve Patient-Centered Medical Home processes and quality measures.

Requirements:

  • Medical Assistant Training with 1+ years of experience in a community health setting or family practice.
  • LPN with experience in ambulatory care or a BA/BS/BSW in a health-related field with 1+ years of experience in community health.
  • CPR certification within 90 days of hire date.
  • NCQA Certification is a plus.
