Job Summary:

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team at Advocate Healthcare of East Boston. As a LPN, you will provide direct personal care to residents, observe and monitor resident health and vitals, maintain accurate documentation, and collaborate with Registered Nurses and Unit Managers on care plans and medical histories.

Responsibilities:

  • Provide direct personal care to residents, including bathing, dressing, and grooming
  • Observe and monitor resident health and vitals, reporting any changes or concerns to Registered Nurses and Unit Managers
  • Maintain accurate and up-to-date documentation of resident care and medical histories
  • Collaborate with Registered Nurses and Unit Managers to develop and implement care plans
  • Participate in interdisciplinary team meetings to discuss resident care and progress

Requirements:

  • Current licensure as a Licensed Practical Nurse in the state of MA
  • Long-term care experience preferred
