Job Summary

We are seeking a highly skilled Youth Mental Health Case Manager to join our team at PermiaCare. The successful candidate will provide evidence-based services to children and their families, assisting them in maintaining or improving their ability to function in the community and work towards their recovery goals.

Key Responsibilities
  • Provide case management and rehab services, including assessment, service plan development, coordinating services, monitoring advocacy and family education.
  • Develop and review recovery plans, ensuring they incorporate the findings of the intake assessment, uniform assessment, and individual choices of the youth.
  • Provide skills training with youth face-to-face, as well as parent skills training.
  • Assess individual needs and develop, implement, and review recovery plans in collaboration with other community resources.
  • Provide crisis services and problem-solving assistance to caseloads, as well as families.
  • Complete Uniform Assessments and Wraparound plans, ensuring accurate and timely documentation.
  • Maintain confidentiality of sensitive records and treatment information, adhering to HIPAA regulations.
  • Establish and maintain effective work relationships with individuals served, supervisors, co-workers, and visitors.
Requirements
  • A Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
  • Previous experience in the field of mental health or social services is preferred.
  • Must maintain a valid Texas Driver's license, auto liability insurance, and a driving record acceptable to PermiaCare's insurance requirements.
  • Required to pass criminal history and background checks, as well as a pre-employment drug screen.
  • Must obtain QMHP certification within 6 months of hire.
Working Conditions

This position requires working independently, under limited supervision, with regular attendance and dependability. The successful candidate will be required to maintain a safe and clean working environment, adhere to company policies and procedures, and display professionalism at all times.
