Job Title: Youth Mental Health Case Manager

At PermiaCare, we are seeking a dedicated and compassionate Youth Mental Health Case Manager to join our team. As a key member of our mental health services, you will play a vital role in providing evidence-based services to children and their families, helping them maintain or improve their ability to function in the community and work towards their recovery goals.

Key Responsibilities:
  • Provide case management and rehabilitation services, including assessment, service plan development, coordination, monitoring, advocacy, and family education.
  • Develop and review recovery plans, ensuring they incorporate the findings of intake assessments, uniform assessments, and individual choices of the youth, as well as clearly identifying client needs, goals, and strategies for achieving goals.
  • Ensure service plans and recovery plan reviews clearly show progress or lack of progress for the required period.
  • Assess individual needs and develop, implement, and review recovery plans, working with other community resources to meet and achieve individual and family service needs.
  • Provide crisis services and problem-solving assistance to caseloads, as well as families, addressing issues such as risk of harm, crisis intervention, and resolution, substance abuse, family/relationship differences, limited community involvement, and individual understanding of illness.
Requirements:
  • A Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education, or early childhood intervention, or a bachelor's degree with at least 30 hours of coursework in the previous fields.
  • Previous experience in the field of mental health or social services is preferred.
  • Must maintain a valid Texas Driver's license, auto liability insurance, and a driving record acceptable to PermiaCare's insurance requirements.
  • Required to pass criminal history and background checks, as well as a pre-employment drug screen.
  • Must obtain QMHP certification within 6 months of hire.
