Job Summary

We are seeking a skilled and compassionate Nurse to join our team at PermiaCare. As a key member of our mental health team, you will be responsible for providing high-quality patient care and support to individuals with severe mental illness.

Key Responsibilities
  • Assess and observe patients, providing interventions and recording details of patient medical history and current health.
  • Prepare patients for treatment and administer medications as prescribed by physicians.
  • Monitor patients for side effects and reactions, and assist in medical procedures as needed.
  • Operate and monitor medical equipment, and relay clinical inquiries from patients, staff members, pharmacies, and insurance companies to the psychiatrist.
  • Educate patients and family members on treatment and care plans, and answer questions regarding medication, alcohol use, tobacco use, and mental health.
  • Document all contacts with patients in a timely manner, and maintain accurate medical information for all patients.
  • Give patient injections as prescribed by physician, and accurately maintain medication/sample inventory.
  • Maintain security and procedures of medication room, and draw labs when ordered by psychiatrist.
  • Call in medication and dispose of medication according to policy and rules, and meet unit performance measures or targets.
  • Maintain assigned caseload of individuals with mental illness, and coordinate services to designated caseload.
  • Enter accurate and appropriate documentation of services within timeframe required, and maintain confidentiality of sensitive records and treatment information.
  • Maintain regular and reliable physical on-site attendance, and comply with the Abuse, Neglect, and Exploitation policy and reporting requirements.
  • Adhere to the Code of Conduct and Standards of Behavior policy requirements, and establish and maintain effective work relationships with individuals served and their families, supervisors, co-workers, and visitors.
Requirements
  • Education: Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in the State of Texas.
  • Experience: At least 1 year experience in psychiatric nursing.
  • Registration, Certification, Licensure or other Qualifications: Must maintain a valid Texas Driver's license, auto liability insurance, and a driving record acceptable to PermiaCare's insurance requirements.
  • Must pass criminal history and background checks, as well as pre-employment drug screen.
  • Current and valid Texas LVN or RN license.
Working Conditions

This position requires the ability to work independently, under limited supervision, and to report major activities through periodic meetings. The Nurse will work in a fast-paced environment, providing care to individuals with severe mental illness. The position requires the ability to work effectively with diverse cultures and socio-economic populations, and to maintain confidentiality of sensitive records and treatment information.

The Nurse will be required to drive a personal or company vehicle, and may be required to travel to agency program sites, community and residential sites, and/or locations outside the PermiaCare catchment area. The position requires the ability to work cooperatively and productively with supervisor, individuals, co-workers, and groups of persons at all levels of activity, contributing to a spirit of teamwork.

The Nurse will be required to maintain valid and current driver's license, auto insurance, acceptable driving record, and reliable transportation at all times. The position requires the ability to acquire and utilize new skills as the job requires, and to maintain highly confidential information.

The Nurse will be required to work in a safe and clean working environment, complying with procedures, rules, and regulations. The position requires the ability to remain calm and professional in stressful situations, and to display professionalism when representing PermiaCare and the program in the community.

The Nurse will be required to maintain compliance with legal requirements and company policies and procedures, and to complete all training as assigned prior to due date.
