As an integral part of the Westwood Medical Specialty Suite, you will deliver extensive psychosocial support to individuals facing intricate medical challenges and recurrent hospital visits.


Key Responsibilities:

  • Manage cases and provide therapeutic counseling
  • Recognize patients and cater to their varied requirements
  • Facilitate referrals for a range of services
  • Revise practice resources and participate in medical consultations
  • Engage in program innovation and educational initiatives
  • Provide on-call support and implement assessment tools
  • Collaborate with healthcare teams to enhance patient well-being
  • Lead therapeutic support groups

Required Qualifications:

  • Master's degree in social work and a valid California license
  • Understanding of psychosocial challenges in patients with complex health issues
  • Expertise in individual, group, and family therapy techniques
  • Competence in database management and electronic health records
  • Ability to collaborate effectively within a team and adjust to evolving priorities
  • Excellent interpersonal skills and a compassionate approach towards patients and colleagues
  • Preferred experience in healthcare settings and collaboration with community organizations
