Behavioral Health Coordinator

In the role of Case Manager, you will be the primary point of contact for clients and families, providing initial assessments and determining their needs. This involves coordinating with our transdisciplinary team to ensure clients receive appropriate services, including mental health therapy, ABA, occupational therapy, speech therapy, and diagnostics.
You will develop individualized care plans, monitor client progress, and provide ongoing support and guidance to clients and their families. Maintaining accurate and confidential records is also a crucial aspect of this position.
As a vital member of our team, you will participate in team meetings and contribute to the development of comprehensive treatment plans. With our client-centered approach, you will have the opportunity to make a meaningful difference in the lives of individuals and families in Thornton and surrounding communities.
