The estimated annual salary for this role is $70,080 - $99,120, based on a 40-hour workweek at $34.00 - $47.00 per hour.


Job Description

We are seeking highly motivated Nursing Care Coordinators to provide comprehensive medical care and support to individuals with mental health conditions in our PHP/IOP setting.


Responsibilities
  • Comprehensive Assessments: Conduct thorough evaluations of patients' physical and mental health status.
  • Vital Sign Monitoring: Regularly assess and monitor vital signs for any signs of medical or psychiatric emergencies.
  • Nursing Care Interventions: Develop and implement personalized treatment plans, ensuring the highest level of care for each patient.
  • Treatment Plan Collaboration: Work closely with psychiatrists, therapists, and other healthcare professionals to develop and implement individualized treatment plans.
  • MEDication Management: Obtain prior authorizations for medications as needed.
  • Patient Education: Provide essential education to clients and their families about mental health conditions, medication management, and coping strategies.
  • Documentation: Maintain accurate and timely documentation of client assessments, treatment plans, and progress notes.

Qualifications
  • Licensure: Licensed or possess the ability to obtain licensure as an Advanced Practice Nurse in the State of Colorado.
  • Education: Professional knowledge of nursing theory and practice at a level normally acquired through the completion of an accredited School of Nursing.
  • Software Proficiency: Proficient in Microsoft Office applications and ability to learn department and job-specific software systems.
  • CPR Certification: Current CPR certification required.
  • Experience: Direct clinical experience working with clients in a setting like PHP or IOP is preferred.
