Job Summary:

The Clinical Care Coordinator is responsible for providing administrative and clinical services to patients in a substance abuse treatment setting. This role requires a strong understanding of substance use disorders and the ability to work effectively with patients, staff, and external partners.

Key Responsibilities:

  • Ensure the maintenance of quality care and monitor treatment of all patients.
  • Conduct internal program review, patient records review, and educational counseling related to substance abuse disorders.
  • Assess training needs of clinical and support staff and report to the Program Director.
  • Ensure staff compliance with Federal and State regulations governing the treatment of patients.
  • Attend conferences, meetings, and training programs as needed.
  • Provide intake/readmission services, follow-up calls, and crisis intervention/supportive services to patients.
  • Collect patient program fees and maintain financial accountability.
  • Develop professional relationships with patients and function as a team leader.
  • Participate in and/or help perform monthly Clinic In-service Trainings.

Qualifications:

  • LADC with Masters degree in behavioral health, and/or LCMHC required.
  • Appropriate work experience in the behavioral health field, with at least five (5) years experience in substance abuse and one (1) year of experience in administering a substance abuse treatment agency.
  • Satisfactory drug screen and criminal background check.
