Key Responsibilities- Assess patient needs and develop personalized care plans in accordance with evidence-based practice, State and Federal regulations, and VNAHSR policies.
- Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective patient care.
- Collaborate with healthcare teams, including physicians, providers, and vendors, to coordinate patient care and ensure seamless transitions.
- Document patient care and progress accurately and thoroughly, adhering to agency policies and procedures.
- Participate in peer review audits and provide constructive feedback to ensure high-quality patient care.
- Advocate for patient care issues and ensure that overall quality and type of care is sensitive to each specific patient's needs.
- 2+ years of experience as a Home Health RN
- State license to practice as a Registered Nurse
- Ability to work independently and as part of a team
- Excellent communication and documentation skills
