Job Summary

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to join our team as a Hospice Care Coordinator. This is a unique opportunity to make a meaningful difference in the lives of our clients and their families.

The ideal candidate will have a strong background in hospice care and a passion for delivering high-quality patient-centered care. As a Hospice Care Coordinator, you will be responsible for providing comprehensive care to clients in their homes, including administering medication, wound care, and other treatments as needed.

We offer a competitive compensation package, including a $10,000 retention bonus for full-time employees. Our team is dedicated to providing exceptional care and support to our clients, and we are looking for someone who shares our commitment to excellence.

Responsibilities:

  • Providing comprehensive care to clients in their homes, including administering medication, wound care, and other treatments as needed.
  • Monitoring clients' conditions and reporting changes to the Clinical or Client Services Manager.
  • Following up with, executing, and properly documenting doctors' orders.
  • Performing client assessments as necessary.

Requirements:

  • A current license as a Registered Nurse in VT.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care and/or hospice experience strongly preferred, but not required.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
