Job Summary

The State of New York is seeking a highly skilled Habilitation Specialist 1 to join our team. As a Habilitation Specialist 1, you will play a critical role in supporting individuals with intellectual disabilities, providing them with the necessary tools and resources to lead fulfilling lives.

Key Responsibilities
  • Meet with and evaluate individuals diagnosed with intellectual disabilities to develop effective habilitative program plans.
  • Train direct support staff on the implementation of habilitation outcomes and documentation procedures.
  • Monitor program planning outcomes through observation and discussion with direct support staff.
  • Maintain accurate records for all assigned cases, including care coordinator and advocate contacts.
  • Facilitate meetings with residential teams and care coordinators to ensure seamless support.
  • Liaise with staff in the Financial Benefits and Entitlements Management Office for related matters.
  • Generate all necessary documents in accordance with Home and Community-based Waiver Services.
  • Adhere to all standards relevant to Medicaid Program relating to HCBS delivery.
Requirements
  • Currently serving as a permanent Habilitation Specialist 1 or have one year of service in a title eligible for a 70.1 transfer in accordance with Civil Service law.
  • A bachelor's or higher-level degree in a human services field, including professional disciplines such as audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, social work, speech/language pathology, and therapeutic recreation.
  • Related academic disciplines associated with the study of human behavior, such as gerontology, human services, sociology, speech communication, or human skills development, including counseling, education, and human development.
