At Stratford VNA, we are seeking a skilled Licensed Practical Nurse to join our team of healthcare professionals. As a Licensed Practical Nurse, you will play a vital role in providing direct patient care under the supervision of a registered nurse.
Key Responsibilities:- Implement the plan of care initiated by the registered nurse, as delegated.
- Provide direct patient care as defined in the Connecticut State Nurse Practice Act, including IV care and maintenance, injections, dressing changes, and patient education.
- Communicate with the primary care nurse and/or supervisor regarding patient information and updates.
- Participate in the coordination of home health services with members of the interdisciplinary team.
- Use equipment and supplies effectively and efficiently, and help arrange for supply and DME acquisition as needed.
- Participate in agency weekend and holiday program, and attend all nursing inservices and meetings as scheduled.
- Graduate of an accredited practical nurse or vocational nursing program.
- Minimum one year nursing experience, with community/home health or medical/surgical experience preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the State.
- Possesses and maintains current CPR certification.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
