Hospice Case Manager Professional

MaxServe Hospice seeks a skilled RN Hospice Case Manager to administer nursing care for patients in their residence, coordinate care with the interdisciplinary group, patient, family, and referring agency, and assume responsibility for care coordination.
Key Responsibilities:
- Administer nursing care for patients of all ages in their residence.
- Coordinate care with the interdisciplinary group, patient, family, and referring agency.
- Assume responsibility for care coordination.
Requirements:
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- Two years experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
- Nursing skills as defined as generally accepted standards of practice.
- Good interpersonal skills.
- Specialized skills preferred in palliative and end-of-life.
- Reliable transportation with valid and current auto liability insurance.
Working Conditions:
- Works in patients' homes in various conditions.
- Possible exposure to blood and bodily fluids and infectious diseases.
- Ability to work a flexible schedule.
- Ability to travel.
- Some exposure to unpleasant weather.
- PRN emergency call.
Physical and Mental Effort:
- Prolonged standing and walking are required.
- Ability to lift up to 50 lbs. and move patients.
- Requires working under some stressful conditions to meet deadlines and patient needs.
- Requires hand-eye coordination and manual dexterity.
