Hospice Care Specialist

Millenia Hospice is seeking a dedicated and compassionate Registered Nurse to join our team. As a Hospice Care Specialist, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.
Key Responsibilities:
- Develop and implement individualized patient care plans
- Monitor and address changes in patient symptoms and conditions
- Collaborate with healthcare teams to ensure comprehensive care
- Provide emotional support and education to patients and families
- Maintain accurate and detailed patient medical records
Requirements:
- Current RN licensure and BLS certification
- Previous hospice experience preferred
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment
