Nurse Navigator

We are seeking a highly skilled and compassionate Nurse Navigator to join our team at Lankenau Medical Center. As a key member of our interdisciplinary team, you will play a vital role in coordinating the clinical care of patients in our oncology program.
Key Responsibilities- Coordinate patient care and develop personalized treatment plans in collaboration with physicians, imaging staff, and other healthcare professionals.
- Act as a patient advocate and liaison between inpatient and outpatient teams to ensure seamless care transitions.
- Participate in program development, quality initiatives, and budget preparation to drive excellence in patient care.
- Develop and implement education programs to enhance patient understanding and empowerment.
- Three years of oncology experience, with one year of oncology nursing preferred.
- Strong verbal, written, and presentation skills, with the ability to motivate and educate patients and families.
- Excellent interpersonal skills, with the ability to interact effectively with diverse stakeholders.
- Current license to practice nursing in the Commonwealth of Pennsylvania or a multi-state license verified through the Nursing Licensure Compact.
- Current certification in oncology nursing by ONC preferred.
- A competitive salary range of $86,000 to $135,012.80.
- Up to 144 hours of paid time off per year, based on full-time or part-time status.
