Clinical Pharmacist Specialist Ambulatory Care

We are seeking a highly skilled Clinical Pharmacist Specialist to join our Ambulatory Care team at Southcoast Health. As a key member of our team, you will play a critical role in providing exceptional patient care and contributing to the development of best practices in pharmacy services.
Responsibilities- Establish and maintain liaison relationships with medical and office staff to ensure safe and effective use of medications.
- Assess for optimal drug product selection, dosage regimen design, and effective administration of medications.
- Provide Medication Reconciliation and Medication Therapy Management tasks as needed, documenting on appropriate media and seeking reimbursement where possible.
- Monitor medication therapies and work with practice staff to identify alternatives, ensuring provision of best possible evidence-based care.
- Provide education to staff and support to pharmacy program management, assisting in the development of best practices and maintenance of cost-effective pharmacy programs.
- Provide one-on-one consultations in an office setting and at patient homes if indicated.
- Bachelor's degree in Pharmacy or PharmD degree required.
- Valid Pharmacist license in the Commonwealth of Massachusetts or obtainable before initiation of employment.
- Specific knowledge and training in prescription drugs, pharmacy benefits, utilization review, cost containment strategies, and emerging technology.
- Minimum of 2 years experience functioning as a clinical pharmacist in the area of practice or completion of a pharmacy residency focused in the area of practice.
- Effective oral and written communication skills.
- Skilled in patient engagement strategies and techniques.
- Highly self-motivated.
- Certification in MTM required in 6 months.
- Certification to administer immunizations (immunization certification) must be obtained within 6 months.
- Disease state management or population-based certification in Diabetes, Cardiovascular, Respiratory, Geriatrics, Psych, Pain, Anticoagulation.
- Experience working in care teams.
- Ambulatory care experience, long-term care experience, or other clinical experience.
Southcoast Health offers a competitive pay range of $119,000 to $149,780.80 per year, depending on level of experience. We also offer a comprehensive benefits package, including a culture of well-being, competitive pay, and opportunities for professional growth and development.
