Position Overview:

Join Bluestone Physician Services as a Care Coordinator, where you will play a vital role in supporting the Special Needs Basic Care (SNBC) population, specifically individuals aged 18-64 residing in community settings and group homes.

This role allows for remote work, with regular travel within your local area to address the needs of your members and the community.


Schedule:
Full-time position with day shift hours, excluding evenings, weekends, and holidays. Standard hours are from 8 AM to 5 PM, Monday through Thursday, and 8 AM to 3 PM on Fridays.

Location:
Norman County, primarily serving the Crookston, Ada, and Fosston areas in Minnesota.

Salary Range:
$72,000

Key Responsibilities:

As a Care Coordinator, you will oversee member caseloads within your designated geographic area. Your responsibilities will include:

  • Coordinating in-person visits with members.
  • Conducting annual assessments, including Personal Care Assistance (PCA) evaluations for community members and customized living assessments for those in Assisted Living facilities.
  • Reviewing current health requirements, setting goals, and creating personalized care plans.
  • Connecting members with community and state resources and services.
  • Completing necessary documentation accurately.
  • Collaborating with medical teams to ensure adherence to healthcare quality standards and utilizing management tools to achieve value-based objectives.
  • Supporting members during transitions of care and working with their care teams to ensure safe discharges and follow-up plans.

Qualifications:

To be considered for this role, candidates must possess:

  • A current Registered Nurse license in Minnesota.
  • A minimum of one year of experience in case management or care coordination, preferably with the elderly or certified disabled populations in settings such as Home Care, Group Homes, Nursing Homes, or Assisted Living.
  • A valid driver's license.
