Position Overview

Bluestone Physician Services is dedicated to delivering outstanding outcomes by providing exceptional healthcare to individuals facing complex, chronic conditions and disabilities. Our innovative care model transcends traditional primary care, as our multidisciplinary teams collaborate with patients, their families, and other healthcare professionals to offer preventative, proactive, and personalized care tailored to individual needs.

In addition to primary healthcare services, Bluestone has developed a comprehensive care coordination model that supports over 14,000 seniors and individuals with disabilities across Minnesota. Our Care Coordination team collaborates with Minnesota health plans to assist members receiving medical assistance through programs designed for special needs and senior health options. Care Coordinators, who are either registered nurses or licensed social workers, engage directly with members to evaluate their physical, mental, and social requirements, facilitating communication and services across their care teams to ensure optimal support and address any gaps in care.

Key Responsibilities:

  • Manage a caseload of members within a designated geographic area.
  • Conduct face-to-face visits and annual assessments, including Personal Care Assistance evaluations for community members.
  • Review health needs, set goals, and create individualized care plans.
  • Connect members with community and state resources and services.
  • Complete necessary documentation accurately.
  • Collaborate with medical teams to meet healthcare quality standards and utilize management tools to achieve value-based objectives.
  • Support members during transitions of care, ensuring safe discharges and follow-up plans.

Qualifications:

  • Must possess a valid Minnesota Licensed Social Worker or Registered Nurse license.
  • Experience working with the elderly or certified disabled populations in care coordination or case management settings is preferred.
  • A valid driver's license is required.
