**Job Overview**

 

We are seeking a dedicated Care Coordinator Specialist to join our team at YAI. As a Care Coordinator Specialist, you will play a vital role in providing case coordination and administrative support to individuals with intellectual and developmental disabilities.

 

**About the Position**

 

This is a part-time opportunity to work within our Residential programs in Westchester County, NY. The successful candidate will have excellent interpersonal, problem-solving, organizational, presentation, and communication skills.

 

**Key Responsibilities**

 

* Manage a caseload of around 75+ individuals

* Input services and meeting minutes into E-Vero

* Take notes in meetings

* Communicate effectively with all families/residences despite challenges

* Understand the clinical needs of clients

* Liaise between families/residences/clinicians/care managers

* Support BIS and other staff/clinicians with client care

* Maintain records in accordance with OPWDD regulations

* Maintain compliance of program in accordance with OPWDD regulations

* Assist with audits

* Assess and implement staff action plans

* Enter and manage databases

 

**Education and Experience**

 

* BA preferred

* Experience working with people with intellectual and developmental disabilities and advocating for their needs/desires required

 

**Compensation**

 

$37,000 - $41,000 per year (based on 20 hours/week)
