As a Case Manager, you will work with medically complex Medicaid members, utilizing an integrated utilization management and case management model that includes assessment, planning, implementation, care coordination, monitoring, and advocacy.
The ideal candidate will have current New York State licensure as a Registered Nurse and certification in Case Management within 24 months of hire. You should also have at least 3 years of recent clinical and Case Management experience, specifically in a Medicaid Long Term Support Program or Health Home setting. We are looking for someone who can demonstrate good judgment when dealing with emotionally charged situations and has the ability to communicate effectively with diverse populations.
Your key responsibilities will include:
- Utilizing your skills to identify medical and psycho-social needs of designated members and provide appropriate education, coordination of care, and resource allocation.
- Engaging individual members and communicating with an established interdisciplinary team to develop a time-tasking tool and an individualized person-centered plan of care.
- Providing guidance on understanding benefit coverage and navigating the healthcare delivery system.
