Emergency Department Navigator with Penn State Health

PennState Health is seeking a skilled Registered Nurse to join our team as an Emergency Department Navigator. This is a unique opportunity to leverage your nursing expertise and passion for patient care to drive exceptional outcomes in a dynamic environment.
About the RoleAs an Emergency Department Navigator, you will serve as a clinical resource, coordinating care and facilitating patient transitions throughout the healthcare continuum. Your strong communication skills and ability to work collaboratively with interdisciplinary teams will be essential in ensuring seamless care delivery.
Key Responsibilities:- Develop and implement effective care coordination strategies to optimize patient outcomes
- Serve as a liaison between emergency department staff, patients, and families to ensure timely and appropriate care
- Utilize advanced knowledge of health systems and resources to identify opportunities for improvement
- Foster collaborative relationships with healthcare providers, payers, and community organizations to enhance care pathways
- RN licensure in good standing
- AHA BLS certification
- Minimum 2 years of experience in emergency nursing or related field
- Exceptional communication and interpersonal skills
- Competitive salary: $95,000 - $105,000 per year
