Healthcare Provider

Responsibilities:
* Provide primary and urgent care services to residents in collaboration with other medical providers
* Work collaboratively with Center Leadership to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion
* Collaborate with the Medical Director and/or Attending Physician for patient-specific medical care
Qualifications:
* Graduate from an accredited School of Nursing program
* Licensed as a registered nurse
* Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner
* Minimum of two years of experience working with adult or geriatric population
* APRN with an unrestricted professional license and DEA for each state in which they practice
