Job Summary:

Nightingale's List is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in rehabilitation settings. As a key member of our healthcare team, you will administer medications, monitor patient health, and develop individualized care plans to ensure the well-being of our patients.

Key Responsibilities:

  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient health by assessing vital signs, performing wound care, and documenting patient conditions.
  • Assist in the development and implementation of individualized patient care plans.
  • Provide bedside care, supporting patients with activities of daily living as needed.
  • Observe and report any changes in patient conditions or concerns to the supervising RN.
  • Follow infection control protocols to maintain a clean and safe environment.
  • Support patients with mobility needs, including turning, repositioning, and transferring.
  • Educate patients and families on healthcare instructions and follow-up care.

Requirements:

  • Must have an active LPN license in the state of New Hampshire.
  • Minimum of 1 year of LPN experience, preferably in correctional, nursing home, or rehabilitation settings.
  • CPR certification is required; additional certifications are a plus.
  • Excellent communication skills and ability to collaborate effectively with a healthcare team.
  • Empathetic, detail-oriented approach to patient care.
