As a Community Habilitation staff member, you will play a vital role in supporting individuals with intellectual and developmental disabilities to achieve their goals and live independently. You will provide one-on-one support, teach life skills, and facilitate recreational activities to promote community integration and empowerment.

Key Responsibilities:

  • Adhere to organizational policies, procedures, and regulations.
  • Utilize effective communication and time management skills to ensure seamless support.
  • Provide individualized support in community and home settings, promoting independence and self-advocacy.
  • Complete mandatory training programs, such as AMAP, CPR/First Aid, SCIP-R, and Defensive Driving, and apply learned skills in practice.
  • Demonstrate OPWDD Core Competencies and ensure the health, wellness, and safety of individuals supported.
  • Manage crisis situations and provide appropriate supervision to ensure a safe and therapeutic environment.
  • Transport and escort individuals supported on recreational activities, medical appointments, and home visits.
  • Foster positive relationships and maintain a therapeutic environment through Personal Outcome Measures, Person-Centered Thinking, and Informed Decision Making.
  • Ensure person-centered plans are implemented and completed documentation for compliance and billing purposes.
  • Utilize foundational teaching skills, such as EMOE, Task Analysis, Essential Skills, Supports, and Modifications.

Requirements:

  • High School Diploma or GED preferred.
  • Work or life experience within the intellectual/developmental disabilities field is an asset.
  • Ability to support individuals with intellectual/developmental disabilities.
  • Basic written and verbal communication skills.
  • Basic computer knowledge.
  • Valid Driver's License may be required depending on the position.
  • Tuberculosis test mandatory for most positions.

Additional Skills and Abilities:

  • Interpersonal and problem-solving skills.
  • Willingness to take initiative and adapt to changing situations.

Work Schedule:

  • 20 hours/week Part-time.
