Role Summary:

The Hospice Aide collaborates with the Clinical Manager and is an essential member of the healthcare team. This role involves providing personal care and related services in a home setting. The Hospice Aide is assigned to specific patients and operates under the guidance and supervision of the Staff Registered Nurse/Case Manager, Supervising Therapist, and Clinical Manager.

Qualifications for Hospice Aide:

  • Current CNA certification in the State of Michigan.
  • At least 1 year of experience as a Hospice Aide in a healthcare or home care environment.
  • CPR certification is preferred but not mandatory.
  • Valid driver's license with current automobile insurance and reliable transportation.
  • Strong communication skills.
  • Adherence to accepted standards and practices.
  • Ability to meet the physical demands of patient care, including ambulation and assistance with exercises.
  • Self-motivated with the capability to work independently.
  • Team-oriented with a consistently positive attitude that fosters collaboration and optimal performance.
  • Must exhibit empathy, nonjudgmental behavior, tact, responsibility, adaptability, organization, and commitment to the agency's goals.

Responsibilities of the Hospice Aide:

  • Help maintain a clean, safe, and healthy environment.
  • Assist patients with mobility, employing safe transfer techniques.
  • Support patients with nutritional and hydration needs, including meal preparation and feeding.
  • Monitor and document vital signs such as temperature, pulse, and respiration.
  • Implement basic infection control procedures.
  • Follow the Hospice Aide care plan diligently.
  • Provide emotional and psychological support to patients and their families.
  • Build trust and confidentiality with patients and their households.

Accountabilities of the Hospice Aide:

  • Notify the Clinical Manager of any changes to the patient visit schedule due to personal time off.
  • Be flexible with scheduling changes during staffing shortages and accept assignments in emergencies.
  • Comply with all relevant laws, regulations, and agency policies.
  • Organize daily visit schedules to optimize travel efficiency.
  • Adhere to OSHA and infection control protocols.
  • Perform additional duties as assigned.
