Role Summary:

The Hospice Aide is accountable to the Clinical Manager and is expected to operate as an integral member of the healthcare team. This role involves delivering personal care and associated services within the home environment. The Hospice Aide is designated to specific patients and works under the guidance and supervision of the Staff Registered Nurse/Case Manager, Supervising Therapist, and Clinical Manager.

Essential Qualifications for Hospice Aide:

  • Current CNA certification in the State of Michigan.
  • At least 1 year of experience as a Hospice Aide in a healthcare or home care environment.
  • CPR certification is preferred but not mandatory.
  • Valid driver's license with current automobile insurance and reliable transportation.
  • Strong communication skills.
  • Adherence to accepted standards and practices.
  • Ability to meet the physical demands of patient care, including ambulation and transferring patients.
  • Self-motivated with the capacity to work independently.
  • Team-oriented with a consistently positive attitude that encourages optimal performance.
  • Must exhibit empathy, nonjudgmental behavior, tact, responsibility, adaptability, organization, and a commitment to the agency's mission.

Responsibilities of the Hospice Aide:

  • Assist in maintaining a clean, safe, and healthy environment.
  • Support patients with ambulation using safe techniques.
  • Assist with nutritional and hydration needs, including meal preparation and feeding.
  • Monitor and document vital signs such as temperature, pulse, and respiration.
  • Implement basic infection control procedures.
  • Follow the Hospice Aide care plan diligently.
  • Provide emotional and psychological support to patients and their families.
  • Build trust and confidentiality with patients and their households.

Accountabilities for the Hospice Aide:

  • Notify the Clinical Manager of any changes to the patient visit schedule due to personal time off.
  • Be flexible with scheduling during staffing shortages and accept assignments during emergencies.
  • Comply with all relevant laws, regulations, and agency policies.
  • Organize daily visits to optimize travel efficiency and maintain productivity.
  • Adhere to OSHA and infection control policies.
  • Perform additional duties as assigned.
