Job Summary

We are seeking a compassionate and skilled Contingent Hospice LPN to provide one-to-one care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide high-quality, patient-centered care to patients in their homes
  • Assess patient needs and develop individualized care plans
  • Implement and evaluate care plans to ensure optimal patient outcomes
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Document patient information and maintain accurate records
Requirements
  • Graduate of an accredited or state-approved school of practical nursing
  • Licensure as an LPN in the State of Georgia
  • A minimum of one year of experience in an acute care setting; hospice or home care experience preferred
  • Phlebotomy and wound care experience required
  • Ability to pass National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications
