Job Summary

We are seeking a skilled and compassionate Contingent Hospice RN Case Manager to join our team at St. Mary's Home Care and Hospice in Athens, GA. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to our hospice clients and their families.

Responsibilities
  • Assess and develop individualized care plans for hospice patients, in collaboration with the interdisciplinary team and physician.
  • Provide direct nursing care to patients, including symptom management, wound care, and medication administration.
  • Collaborate with the hospice team to identify and address patient and family needs, including grief and loss issues.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in weekly interdisciplinary team meetings to discuss patient progress and develop care plans.
Requirements
  • Graduate of an approved nursing education program.
  • Licensure as a Registered Nurse in the State of Georgia.
  • One (1) year experience as a professional acute care nurse.
  • Home care experience preferred but not required.
